67-amino-acid growth factor structurally related to IGF-1 and insulin. Acts at the IGF-1 receptor and the IGF-2/cation-independent mannose-6-phosphate receptor. Critical for prenatal growth; in adults, plays roles in muscle stem cell function and tissue regeneration.

Best timing
Limited human data on supplementation. Pre-clinical research uses daily dosing, often around muscle injury or training stimulus.
Contraindications
- Active cancer (IGF-2 is overexpressed in many tumors and drives proliferation)
- Pregnancy
- Hypoglycemia history
- Diabetic retinopathy
Watch symptoms
- Hypoglycemia (less than IGF-1 but present)
- Injection site reactions
- Theoretical proliferative risk with chronic high dosing
- Limited human safety characterization outside of native physiology research
